Change Business Information

The Change Business Information form Opens a new window should be used when any of the following items has been changed:

  • Business Name
  • Business Address or Phone Number
  • Business Type

Business Resolution

The Business Resolution form Opens a new window is a required document that identifies the individuals who will be authorized to conduct banking activities for the business. This is required at account opening, and if a business owner would like to add a new authorized signer to an existing account.

Required Account Opening Documents

After submitting your application, you have 60 calendar days to provide us with a few required documents before your account can be approved. Choose your business type from the table below to see what documents are needed.

  2. Proof of Existence Document
    (See options on the right)
  3. Voided Check with the Exact Business Name Preprinted
ONE of the following is required:
  • Copy of Current Business or Occupational License
-OR-
  • Copy of Current Tax Registration Certificate, Permit or Declaration for any State or Local Tax, for example:
    • Sales Tax
    • Use Tax
    • Excise Tax
    • Gross Receipts Tax
    • Documentary Stamp Tax
-OR-
  • Copy of Fictitious or Assumed Name Certificate. Also known as:
    • Trade Name Registration
    • DBA (Doing Business As) Registration
    • Fictitious Name Statement
    • Statement of Trade Name
PARTNERSHIP ALL of the following are required:

  • Copy of Current Constitution or other governing document (Articles of Association, Declaration of Organization, etc)
